Prophetstown Park District is offering Flag Football (K-5th) and Outdoor Soccer (PK 4 year olds - 5th grade) again this year! You can print out your form using the links below, complete it and turn it in to the Erie District Office or the Erie Elementary. Forms are due AUGUST 19th. We have copies of the forms available for pick up at the District Office.
